Mastercare of Kailua-Kona is hiring a Transitional and Recovery Caregiver to help clients return home safely after hospitalization, rehabilitation, illness, or a change in condition. Working from an approved care plan, you will support routines, personal care, dressing, grooming, meal preparation, hydration, light housekeeping, mobility, fall-prevention practices, appointment readiness, and medication reminders. You may help organize supplies, keep essential items within reach, encourage prescribed activities within your training, and accompany authorized errands or follow-up visits when assigned. You will not diagnose, change medications, or provide clinical services unless separately licensed, authorized, and assigned to do so. Caregivers observe for changes in pain, appetite, energy, skin condition, mobility, breathing, behavior, or home safety and report concerns promptly through established channels. Accurate mobile EVV documentation, dependable attendance, confidentiality, and respectful communication with clients, families, supervisors, and authorized providers are required. Experience with recovery, mobility support, chronic conditions, or senior care is helpful, but applicants with transferable skills and willingness to complete training may be considered. Shifts may be short-term or ongoing and may include weekdays, evenings, weekends, or holidays.
